This position is Full-Time, reporting to Registered Nurse Team Lead

We are looking for a Clinical Licensed Practical Nurse. This position will report to the RN Team Lead. This role functions as a remote patient monitoring nurse with an individual patient case load. Accountable for the application of care pathways for remote clinical nursing practice and patient care coordination for chronic disease diagnoses. There is responsibility to manage the resources with mindfulness regarding adherence of regulatory and professional standards, including Health Insurance Portability and Accountability Act (HIPAA) as well as any additional security requirements identified. It is imperative to foster collaborative relationships within all areas of the interdisciplinary workings of the contracted clinic/health care provider and Brook as a partnership.

Key Responsibilities:

  • Conduct comprehensive review of subjective and objective information to determine patients’ eligibility and readiness for remote monitoring.

  • Educate patients and their families or care takers (if applicable) on the program, monitoring devices, and protocols.

  • Educate and instruct patients on how to use their monitoring devices, including frequency of use and initial set up of the device.

  • Assist patients with account creation in the Brook Health app.

  • Ensure all necessary documentation is completed accurately and promptly.

  • Identify onboarding process improvement opportunities to continue driving efficiency and scalability.

  • Work in a patient centered, tech-forward model of healthcare.

  • Collection and review of selected data about the health status of the patient, while providing encouragement to patients, close gaps in care and enter documentation in EMR.

  • Responsible for timeliness of review and quality communication and/or escalation of data.

  • Communicate in an attentive and individualized manner with each encounter, verify patient’s understanding of encounter.

  • Works closely with the Brook Health Care Team, including Lead RN, Health Coaches, Clinic Success Manager, and individual office staff to assure the efficient operation of quality and clinical activities.

  • Ensure accurate and thorough clinical documentation of care coordination and CPT code requirements for all patients.

  • Developing a deep understanding of the patients and members we serve advocating and reinforcing health care goals.

  • Other job-related duties as required.

  • Obtaining information regarding Quality metrics for individual providers for their patients, such as diabetic eye exams, medications changes, specialist follow ups etc.

Qualifications:

  • Unrestricted and Active Practical/Vocational Nursing licensure in NY, MA, CT (requirements to obtain additional licensure as identified): understanding of scope of practice.

  • Primary Care office experience preferred.

  • Knowledge of Gaps in Care including medication reconciliation, barriers to care, determinates to health preferred.

  • Experience with EMR/EHR and technology required for patient communication and access to resources.

  • Strong organizational and personal skills.

  • Strong technical skills: ability to use EMR, access various databases, manage patient health information on multiple platforms.

  • Coordination with patients, primary care providers and specialists.

  • Able to dissemination PCP and specialist information via EMR.

  • Excellent organization skills with the proven ability to work independently.

This position is not eligible for relocation or visa sponsorship. Candidates must live within a commuting distance from the office. This is primarily a remote role, with infrequent onsite time for meetings or training. Salary is commensurate with experience.

The pay range for this role is:

$60,000 - 62,000 USD per year (MA/CT Location for Job Posting)
