Brook Remote Care


We’re here to empower you to live the life you want by providing you with personalized, evidenced-based ways to stay healthy and manage your health. Many factors contribute to health and wellbeing: managing health conditions, stress levels, what you eat, how much you move, and more. We’ll focus on all these factors and more throughout the program. 

The solution to good health lies in finding what works for you, then making and maintaining small changes over time to achieve your health and wellness goals. It’s our job at Brook to help you do that. 


When it comes to managing health, research tells us: 

  • Health is 100% unique to the individual. 

  • Tracking health data such as blood sugar, blood pressure, weight, meals, and more helps you see what works and what doesn’t. 

  • Tracking and adjusting habits based on your personal data helps prevent acute (short-term) and chronic (long-term) complications.

  • Finding the healthy habits that work for your lifestyle make them stick and allow you to achieve success.

  • The “why” you are managing your health is just as important as the “how.” In other words, you need to know your motivations for getting healthy in order to succeed long-term.


We’ll be working with you on a few simple objectives: 

  • Identifying what you’d like your health to look like – how you’d like to look and feel, plus any goals you have such as being able to play with grandchildren or having more energy throughout the day.

  • Understanding any health conditions you have been diagnosed with.

  • Monitoring your health to help you make informed decisions. 

  • Making the small daily decisions that add up to big results for your health and wellness.

We’ll support your health journey in a way that works for you, your schedule, and way of life. We’ll help you set realistic goals and provide you with the tools and support to reach and sustain them. 


There are three keys to success in this program: tracking your progress, working with Brook’s Health Coaches, and your willingness to try.

Tracking your progress:

In order to make smart decisions that are personalized to you and your health, you need data. So it’s important that you connect your devices to the Brook app, and use them regularly. By “devices” we mean health data collection devices such as a glucose meter, blood pressure cuff, or digital scale. It’s also important to log additional data including meals and physical activity, so you and your care team can see the entire picture on your health.

Working with Brook’s Health Coaches:

The second key to success is chatting with the Brook Health Coaches regularly. Our Health Coaches are highly trained nutritionists, dietitians, and other health experts that know how to help people get healthy, no matter their starting point. Brook members who chat with the Health Coaches regularly are more likely to succeed.

Willingness to try:

You get out of Brook what you put in. Our team at Brook knows that with the right support, knowledge, and motivation you can manage your health and lead a healthier lifestyle. We can’t promise it will be easy, but we will be with you every step of the way, offering guidance, knowledge, and evidence-based research to put you on the path of success. This program has helped so many people, and we are confident it can help you, too.


So how does this program work? What will happen day to day?

Your Brook program is personalized for you and your needs. You’ll get devices to track the health data your provider has prescribed in your care plan, and you’ll work with Brook’s Health Coaches to set goals that are right for you around nutrition, sleep, stress, activity, and more. 

Here are some of the key features of the Brook smartphone app you can use on your path to a healthier you:

  • HEALTH COACHES: Chat with your Health Coaches regularly to set goals, ask questions, tackle roadblocks, and celebrate success. You can message them anytime by opening your Brook app, tapping Care Circle in the upper-left corner, then selecting Health Coaches. They’ll get back to you from 8am-9pm EST each day. 


  • AUTOMATED LOGGING: Once you connect your devices to Brook, your health data will log automatically. You can easily log additional data like meals. 


  • MEDICATION REMINDERS: Set up a medication schedule to get reminders to take and log your medications, so you never forget again, and your provider can make sure your dosages are right. Your provider can even make medication changes through the app, so you can feel better right away without the need for an in-person appointment.


  • DATA DASHBOARDS: Want to view your progress for logged weights, blood pressures, and more? Check out your data dashboards by tapping Profile in the upper-right corner of your Brook app, then select the data and timeframe you want to view. Don’t worry – you don’t need to interpret your data, we’ll do that for you.


  • DATA ALERTS: Brook and your provider can get alerts when your readings are out of range. This helps your care team support you when you might need it the most.


  • EVIDENCE-BASED RESOURCES: Brook provides education materials backed by science. We have a wide variety of print-outs, blogs, and structured challenges to meet your needs and your schedule.


  • OFFICE VISIT PREPARATION: Have better and more productive appointments with your provider. Chat with a Health Coach before your appointment to discuss the top questions to ask in-office, and email or print out a snapshot of your recent health data on Brook to discuss your progress and goals.


Brook is designed to help you and your care team better understand your current health status, and find your best path to a healthier you. 

Brook provides:

  • A one-stop shop smartphone app to support your health anywhere, anytime. 

  • A direct connection to your care team and support from Brook’s Health Coaches. No more waiting in-office or on the phone to get your questions answered or medications adjusted. 

  • A clear path to a healthier you. With the right health data, you and your provider can make the right decisions about your health quickly, instead of waiting for your next appointment. 

By the end of the program, your goal is to: 

  • Feel supported in managing your health status. 

  • Learn new skills that help you maintain your health long term.

  • Enjoy increased energy and health to do the things you love! 



Here are some actions you should take before you move onto the next week:

  1. Explore the Brook app

    • Take a moment to explore the Brook app and program if you haven’t already, use our app tutorial by tapping Profile > Your Name > Support > Launch Tutorial to learn more about the ins and outs of the app. You can also go through this while on the phone with one of our onboarding specialists.   2. Get started on Brook

    • Introduce yourself to the Brook Health Coaches by tapping Care Circle then selecting Health Coaches

    • Keep an eye out for new program material each week. 

  3. Turn on Brook app notifications

    • You can turn these on by tapping Profile > Your Name > Permissions > Communicate Permissions. Make sure that push notifications and text messages are toggled on. 

  4. Put the app front and center

    • Add the app shortcut on the homepage of your phone so you see it every time you open your phone as a little reminder
