Job Summary:

The Patient Data Management Coordinator is responsible for managing critical administrative duties to support the acquisition of new patients and the management of active patients in the Remote Care workflow. This role ensures smooth eligible patient identification, order/referral management, active patient management tasks, and billing support to maximize efficiency across multiple clinics and health systems. The ideal candidate is highly organized, detail-oriented, and experienced in healthcare operations, EMR management, and medical billing workflows.

Key Responsibilities

Patient Data and Referral/Order Intake & Processing

  • Collect patient referrals and Remote Care orders from multiple sources, including EHRs, faxed documents, spreadsheets, SFTP, etc.

  • Review orders for completeness and accuracy, verifying required information such as patient demographics, provider details, diagnosis, and Remote Care eligibility criteria.

  • Work closely with Partner Success Managers and internal teams to resolve missing or incorrect information before processing orders.

  • Coordinate with the Remote Care implementation team to ensure a smooth patient onboarding process.

Data Standardization and Uploading

  • Convert and standardize data from various formats (PDF, fax, EHR exports, spreadsheets, SFTP) into a structured format for system compatibility.

  • Accurately input and validate patient and provider data into the Patient Acquisition Platform. Ensure that all data entries comply with HIPAA and company security policies for patient information.

Database Management & Quality Assurance

  • Maintain and update patient referral and order records, ensuring real-time visibility into patient enrollment status prior to Benefits Investigation.

  • Perform routine audits to identify and correct discrepancies in the database.

  • Generate and distribute referral intake reports to Partner Success, implementation teams, and billing departments as needed.

  • Work with Partner Success to ensure order/referral throughput and avoid workflow bottlenecks.

  • Address missing or incomplete orders and follow up with Partner Success as needed.

Eligible Patient Workflow Management

  • Work closely with the Benefits Investigation Team to ensure timely handoff of medically eligible patients.

  • Manage the Deceased/Transferred patients in the Patient Acquisition Platform

  • Flag eligible patients in the EMR to streamline provider decision-making and order placement.

  • Maintain data integrity by regularly reviewing and updating patient records.

  • Generate and manage reports related to eligibility, orders, and throughput metrics.

Active Patient Maintenance & Data Management

  • Field clinic messages/triages and to-do’s for active patients

  • Work with clinic staff and providers to ensure order/referral throughput and avoid workflow bottlenecks.

  • Address missing or incomplete orders and follow up with providers as needed.

  • Update the eligibility status and flag removal in the clinic EMRs

  • Assist in the completion of manual data entry projects to support internal and external reporting and overall data integrity.

Billing & Compliance Support

  • Upload and manage monthly billing reports, ensuring proper documentation for reimbursement.

  • Work with billing teams to ensure accurate charge capture and compliance with CMS guidelines.

  • Identify discrepancies and work to resolve any billing or reporting issues.

Cross-Department Coordination

  • Collaborate with Partner Success Managers, Growth Marketing, Product, and Revenue Cycle Management to optimize workflow efficiency.

  • Provide operational support to ensure seamless provider engagement and patient activation.

  • Support process improvement initiatives to enhance RPM workflow scalability.

Qualifications

  • Bachelor’s degree in healthcare administration, business, or a related field (or equivalent experience).

  • 2+ years of experience in healthcare operations, medical billing, EMR management, or patient workflow coordination.

  • Strong knowledge of EMR systems, patient eligibility tracking, and order management.

  • Experience with insurance verification and benefits investigations in a healthcare setting.

  • Detail-oriented with strong organizational and analytical skills.

  • Proficiency in Microsoft Excel, data entry, and reporting tools.

  • Ability to work independently and manage multiple tasks in a fast-paced environment.

  • Strong communication skills, with the ability to collaborate across departments.

This position is not eligible for relocation or visa sponsorship. Candidates must live within a commuting distance from the office. This is a hybrid role, onsite in the office required weekly along with remote work.

The pay range for this role is:

$62,000 plus variable compensation USD per year (Buffalo Location for Job Posting)
