Job Overview:
This position is Part-Time, reporting to Lead Intake Nurse/Associate Director of Care Team Development.
We have an opening for an Intake Specialist, Licensed Practical Nurse on the Brook Health Care Team. We are seeking a highly skilled and proactive onboarding specialist to provide intake outreach phone calls to onboard patients onto our Brook Remote Care program. In this role, you will play a critical part in the onboarding of patients into the program, showcasing Brook’s benefits and ease of integration of the program into each patient’s daily life. You will explain the program benefits, expectations, develop an initial care plan and help patients take their first device reading. In this position you will act as the face of Brook Remote Care to each patient.
As a key player in our team, you will collaborate with the Intake Team and other cross functional teams acting as a liaison in patient outreach and conversion. The ideal candidate will possess a background in healthcare knowledge, excellent communication skills, and experience in phone outreach to patients.
Key Responsibilities:
Patient Assessment and Onboarding:
Conduct comprehensive review of subjective and objective information to determine patients’ eligibility and readiness for remote monitoring.
Educate patients and their families or care takers (if applicable) on the program, monitoring devices, and protocols.
Educate and instruct patients on how to use their monitoring devices, including frequency of use and initial set up of the device.
Assist patients with account creation in the Brook Health app.
Ensure all necessary documentation is completed accurately and promptly.
Identify onboarding process improvement opportunities to continue driving efficiency and scalability.
Qualifications:
Unrestricted Practical/Vocational Nursing licensure in NY, MA, CT (requirements to obtain additional licensure as identified).
Minimum of 3 years of telephonic triage or clinic outreach.
4 years of patient facing experience and/or telephonic communication, preferred.
Prior experience in positions managing caseloads of remote communication to patients.
Proficient in using outbound calling systems and remote technology.
Technical aptitude to simplify and concisely educate patients about the Remote Care Program.
Strong critical thinking skills.
Excellent interpersonal and communication skills.
Ability to work independently and as part of a multidisciplinary team.
Ability to work in the Eastern Standard Time zone to service the needs of Brook’s patient population.
This position is not eligible for relocation or visa sponsorship. Candidates must live within a commuting distance from the office. This is primarily a remote role, with infrequent onsite time for meetings or training. Salary is commensurate with experience.

The pay range for this role is:
30 - 30 USD per hour (Worcester Location for Job Posting)
30 - 30 USD per hour (Buffalo Office)
